As was requested during the January meeting we are going to mail a letter to the National Division regarding the COVID leave and FMLA. We ask that everyone who had covid leave to participate. You do not have to answer all or any of the questions that are in the questionnaire. However, it will be helpful on making our case.

These two files will help us with our data collection. The form will only accept PDF files. We cannot accept screenshots since they are hard to read.
